Create 3 staff notes specifically addressing how you are ,coordinating services for
the client
o Each staff note needs at least 250 words (who, what, where, when, why, how
client observations)
• Discharge your client from services and create a summary including the following
information
o Reason for discharge (successful, referral, not engaged, etc.) and information
supporting the recommendation
o Progress on goals and objectives
o Plan for the future and follow-ups
o Date
